Community Navigator

Job Summary

As a key component of the PRIDE Navigator Plus grant, Community Navigators are responsible for maneuvering participants through a continuum of services strategically aligned to build their resiliency against the contraction of HIV/AIDS, substance abuse and other risk factors and/or manage existing cases.  The Community Navigators under the direction of the Project Coordinator will serve as key staff in identifying and removing barriers to the participants'   full involvement within the Continuum of Care Services.

 

Duties and Responsibilities

  1. Assist in the creation of Individual Development Plans for participants as guided by the Acuity Scale Tool
  2. Assist in the management and coordination of navigated service delivery sites for participants, as guided by the Individual Development Plan/Acuity Scale
  3. Assist in the identification and removal of barriers to participants fulfilling slated developmental goals and objectives
  4. Assist in the recruitment of participants
  5. Assist with the management of participants i.e. tracking participants and keeping records of participation utilizing Excel spreadsheet, and other tracking measures.
  6. Assist Project Director and Grants Coordinator with developing, implementing and maintaining Social Marketing strategies.
  7. Participate in PRIDE Navigator Advisory Board meetings and program trainings.
  • Complete other duties and activities as needed

Qualifications

Minimum:  List the minimum qualifications required to successfully perform the job and are necessary to be considered for the position.

 

  • Education High School GED
  • Minimum years of experience required (do not include a range of years) 1 year
  • Specialized knowledge community resources, customer service
  • Skills communication (oral and written), activity management
  • Other characteristics such as personal characteristics Must be open-minded and appreciative of diversity
  • GPA NA
  • Class rank (i.e. Junior, Graduate, Post-doc, etc.) or/ NA

 

 

 

Preferred:  State the qualifications that would be beneficial to have, but are not essential to the job.

 

  • Education Some College Experience (Behavioral Health/Social Work)
  • Years of experience  - 2 years
  • Specialized knowledge, Professional Certification(s)  Mental Health Evidenced Based Practices, Agency Mobilization, Diversity & Inclusion,  Proven Experience with the LGBTQQ community preferred
  • Skills Activity Management, Marketing (traditional and social media), Public Speaking

 

Working conditions

The work environment will be the Office of Student Health and Counseling Services, Community

Based Organizational (CBO) Partner Sites, and the University/Community at large.  

 

Physical requirements

Some lifting (up to 10 pounds) may be required when sending or receiving shipments, in handling samples, supplies or literature, and transport of laptop computer.
